Our model of care is to proactively partner with patients to help them obtain their best health through innovative care\, regular check-ups\, and community through our Senior Activity Centers.  We have proudly been serving Corpus Christi since 2013.\n\n\n\n"Conviva Care Centers are proud to be expanding to better serve the community of Corpus Christi.  From the start at our original location on Port Avenue\, we have enjoyed being part of the communities in which we serve.  Now with our new location at Alameda and Everhart\, we are enjoying another community to serve and provide health and wellness to seniors."\n\n\n\nConviva Care Center   Bayside welcomes back Dr. Stephen Barth\, a pillar of the Corpus Christi medical community for over 37 years.  Dr. Barth returns to daily practice after serving for 2 years as medical director for Conviva Care Solutions.  He is very excited to be back caring for his patients and is joined by two excellent nurse practitioners Margo Gonzalez-Hussey and Lynn Van Epps.  We are also thrilled to have moved into the Town & Country Shopping Center\, one of our communities most well-known shopping centers.\n\n\n\nIn attendance will be local dignitaries\, company officers\, and crowd of neighboring community and patients.  The ribbon cutting ceremony will occur in front of these participants.
